Elden Ringplayers should summon a Spirit Ash built like a tank to distract Esgar and take the most damage. While the boss is distracted, the Tarnished can use Rotten Breath to inflict Scarlet Rot and use a shield to protect themselves from incoming attacks while the Spirit Ash regains their attention.
